Job Summary
We are seeking a dedicated and compassionate Registered Nurse specializing in Rehabilitation to join our team. The ideal candidate will possess a strong background in patient care, with experience in various medical settings including acute care and long-term care. This role requires a commitment to providing high-quality rehabilitation services to patients recovering from illness or injury, ensuring their comfort and promoting their recovery.

Duties

• Conduct comprehensive patient assessments and develop individualized care plans.
• Administer medications and treatments as prescribed, including IV infusion and acute pain management.
• Monitor patient progress and adjust care plans as necessary, utilizing EMR systems for documentation.
• Collaborate with interdisciplinary teams to provide holistic patient care, including physical examinations and diagnostic evaluations.
• Educate patients and families on rehabilitation processes, health coaching, and discharge planning.
• Maintain compliance with HIPAA regulations and NCQA standards in all patient interactions.
• Provide support for patients with developmental disabilities, dementia care, and memory care needs.
• Utilize critical care skills when necessary, including airway management and ventilator support.
• Engage in case management activities to ensure continuity of care across various settings such as nursing homes, assisted living facilities, and home health.

Qualifications

• Graduate of an accredited registered nursing program required with a current unencumbered Registered Nurse License in Iowa or from a Compact State.
• One (1) year rehabilitation nursing experience
• Basic Life Support (BLS)
